Abstract
Sulphuric Acid Storage Tank merupakan fasilitas penting dalam industri smelter sebagai penampung Asam Sulfat sebagai hasil samping smelter yang merupakan bahan baku untuk industri petrokimia lainnya. Potensi bahaya dan risiko pada proyek Sulphuric Acid Storage Tank cukup tinggi sehingga diperlukan pengendalian terhadap bahaya dan risiko untuk meminimalkan dampaknya terhadap aspek keselamatan, kesehatan dan lingkungan. Penelitian ini merupakan penelitian kualitatif dengan studi kasus, data dikumpulkan dengan kuesioner, interview dan focus group discussion (FGD). Tujuan dari penelitian ini adalah membuat kerangka manajemen risiko yang terintegrasi dengan proyek Sulphuric Acid Storage Tank dengan mengidentifikasi bahaya dan risiko, menganalisa risiko untuk mendapatkan tingkat risiko dan menentukan faktor yang dapat menyebabkan kegagalan pada proyek. Penilaian tingkat risiko dilakukan dengan metode HIRARC (Hazard Identification, Risk Assessment & Risk Control) sedangkan faktor-faktor penyebab risiko yang paling tinggi dilakukan dengan pendekatan FTA (Fault Tree Analysis). Tingkat risiko yang paling tinggi dari hasil analisis dengan menggunakan metode HIRARC adalah terjatuh dari ketinggian yang terjadi pada proses pekerjaan instalasi shell plate, assembly & instalasi roof plate dan instalasi aksesoris storage tank. Berdasarkan analisis fault tree, terdapat 34 basic event dan setelah dilakukan analisis dengan metode MOCUS (Method for Obtaining Cuts Sets) didapat 28 kombinasi basic event yang dapat menyebabkan risiko terjatuh dari ketinggian.

Sulphuric Acid Storage Tank is important facilities in smelter industry, which store Sulphuric Acid as a by-product of smelter, which is a raw material for other petrochemical industries. The potential hazards and risks in Sulphuric Acid Storage Tank project are quite high, so it is necessary to control the hazards and risks to minimize their impact on safety, health and environmental aspects. This research is a qualitative research with case studies, data collected by questionnaire, interview and focus group discussion (FGD). The objective of this research is to create an integrated risk management in Sulphuric Acid Storage Tank Project by identifying hazards and risks, analyzing risks to obtain risk levels and determining factors that can cause project failure.The risk level assessment carried out using HIRARC (Hazard Identification, Risk Assessment & Risk Control) method while the factors causing the highest risk are carried out using FTA (Fault Tree Analysis) approach. The highest level of risk from the results of analysis using HIRARC method is a fall from a height that occurs in shell plate installation, roof plate assembly & installation and installation of storage tank accessories. Based on fault tree analysis, there are 34 basic events and after analysis using the MOCUS method (Method for Obtaining Cuts Sets), 28 combinations of basic events were found that can cause the risk of falling from a height.
