Abstract
Dari 858 permohonan Izin Mendirikan Bangunan (PBG) di Kota Samarinda, tingkat keberhasilan penerbitannya hanya 4,31%, dengan penolakan 1,63%, 0,93% masih dalam proses di Dinas Perizinan, dan 93,12% dalam proses permohonan di Bagian Teknis. Kantor. Penolakan tersebut terjadi karena ditemukan ketidakkonsistenan dalam permohonan. Selanjutnya, terjadinya backlog dalam proses permohonan disebabkan adanya kendala yang timbul dari setiap status permohonan pada saat proses izin mendirikan bangunan. Perbedaan statistik yang signifikan antara 2 Agustus 2021 dan 6 Februari 2023 menimbulkan kekhawatiran masyarakat umum karena belum sepenuhnya dipahami faktor penyebab kegagalan tersebut, seperti kepatuhan terhadap persyaratan dokumen administrasi dan teknis yang ada. Oleh karena itu, penting untuk memahami secara rinci hambatan-hambatan yang menyebabkan kegagalan dalam memulai proses penerapan PBG. Penelitian ini bertujuan untuk mengidentifikasi secara kualitatif akar permasalahan kegagalan tersebut dengan menggunakan metode Fault Tree Analysis (FTA) untuk mengantisipasi potensi permasalahan pada aplikasi PBG lebih lanjut. Selain itu juga bertujuan untuk mengetahui dampak, penyebab, dan tindakan perbaikan terhadap potensi hambatan yang dapat mengakibatkan kegagalan secara kuantitatif dengan menggunakan metode Failure Mode and Effect Analysis (FMEA) pada kinerja sistem. Evaluasi dan penilaian hasil identifikasi didasarkan pada kriteria tingkat keparahan, kejadian, dan tingkat deteksi sebagai data pendukung yang disesuaikan dengan komponen dan variabel penelitian. Hal ini penting dalam mengatasi keterbatasan informasi bagi pejabat pengelola izin mendirikan bangunan pada saat proses penerbitan PBG. Hasilnya, penelitian ini telah mengidentifikasi 11 kejadian risiko yang dipilih responden dan menemukan 144 penyebab kegagalan beserta alternatif tindakan perbaikannya. Berdasarkan urutan tingkat prioritas, terdapat 22 penyebab kegagalan yang diprioritaskan untuk dilakukan mitigasi melalui penghapusan, pengurangan, atau penerimaan risiko. Salah satu mitigasi utama yang disahkan dan disetujui oleh perwakilan Sekretariat PUPR adalah dengan melakukan sosialisasi secara berkala kepada pemangku kepentingan pengelola gedung dan membuat pedoman penerapan PBG secara detail dan transparan

Out of 858 Building Permit (PBG) applications in Samarinda City, the success rate for issuance is only 4.31%, with 1.63% being rejected, 0.93% still in the process by the Licensing Office, and 93.12% in the application process in the Technical Office. The rejections occur due to inconsistencies found in the application. Furthermore, the backlog in the application process is due to obstacles arising from each application status during the building permit process. The significant difference in statistics between August 2, 2021, and February 6, 2023, has raised concerns among the general public because the factors contributing to these failures are not yet fully understood, such as compliance with the existing administrative and technical document requirements. Hence, it is crucial to understand in detail the obstacles leading to the failure to initiate the PBG application process. This research aims to qualitatively identify the root causes of these failures using the Fault Tree Analysis (FTA) method to anticipate potential issues with PBG applications further. Moreover, it aims to determine the impact, causes, and corrective actions for potential obstacles that may result in failure quantitatively using the Failure Mode and Effect Analysis (FMEA) method within the system's performance. Evaluation and assessment of the identification results are based on severity, occurrence, and detection level criteria as supporting data tailored to the research components and variables. This is important in overcoming the information limitations for building permit management officials during the PBG issuance process. As a result, this study has identified 11 risk events selected by respondents and found 144 failure causes along with alternative corrective actions. Based on the order of priority levels, there are 22 failure causes which are prioritized for mitigation through eliminating, reducing, or accepting the risks. One of the primary mitigations validated and approved by the representatives of the PUPR Secretariat is to conduct regular socializations with building management stakeholders and to create a detailed and transparent guidelines for PBG applications.
