Abstract
Pelabuhan merupakan sebuah fasilitas atau wilayah yang dilengkapi dengan infrastruktur dan perlengkapan yang diperlukan untuk menangani aktivitas bongkar muat kapal, baik kapal penumpang maupun kapal barang, karena adanya berbagai aktivitas di pelabuhan sudah pasti mengandung risiko, khususnya pada permasalahan Keselamatan dan Kesehatan Kerja (K3) kegiatan bongkar muat karena setiap aktivitas pada bongkar muat memiliki bahaya risiko pada kecelakaan kerja. Penelitian ini ditulis untuk menganalis risiko dominan dan mencari akar penyebab kegagalan pada kegiatan proses bongkar muat di Pelabuhan XYZ dengan menggunakan metode FFMEA dan FTA. Hasil analisis FFMEA menunjukkan nilai FFMEA tertinggi dengan nilai 8.071 pada aktivitas mempersiapkan peralatan kerja dengan risiko pekerja terluka. Untuk mengetahui akar penyebab kegegalan tersebut dilakukan analisis dengan metode FTA. Hasil analisis menggunakan metode FTA diketahui penyebab pekerja terluka adalah kondisi lapangan kerja yang kurang memadai dengan nilai probabilitas 0.005882. Penelitian ini juga memberikan rekomendasi untuk meminimalisir risiko yang dapat menyebabkan kegagalan.

A port is a facility or area equipped with the necessary infrastructure and equipment to handle the loading and unloading activities of ships, whether passenger or cargo ships. Due to the various activities at the port, it inevitably involves risks, particularly in terms of Occupational Health and Safety (K3) issues during loading and unloading activities, as each activity in loading and unloading carries the risk of work accidents. This study is written to analyze the dominant risks and identify the root causes of failures in the loading and unloading process activities at Port XYZ using the FFMEA and FTA methods. The FFMEA Analysis results indicate the highest FFMEA value of 8.071 for the activity of preparing work equipment, with the risk of workers getting injured. To determine the root causes of these failures, an Analysis using the FTA method was conducted. The FTA Analysis results revealed that the cause of workers getting injured is inadequate working field conditions, with a probability value of 0.005882. This study also provides recommendations to minimize risks that could lead to failures.
