Abstract
Komite Nasional Keselamatan Transportasi (KNKT) mencatat 147 kecelakaan pelayaran pada rentang tahun 2017 hingga 2023. Kecelakaan kapal terbakar mendominasi jenis kecelakaan yang teridentifikasi, dengan kejadian mencapai 32%. Menurut statistik, lebih dari 75% korban jiwa kecelakaan di laut disebabkan oleh human error. Dalam penelitian ini, penulis melakukan analisis terhadap faktor awak kapal dalam kasus-kasus kebakaran kapal Ro-Ro di Indonesia. Analisis kualitatif dan kuantitatif dilakukan dengan metode Fault Tree Analysis (FTA) untuk menjabarkan kegagalan peran awak kapal dan dampaknya terhadap kegagalan dalam penanganan kebakaran kapal Ro-Ro. Hasil analisis menunjukkan nilai probabilitas kegagalan penanganan kebakaran akibat gagalnya peran awak kapal adalah sebesar 1.02×10^(-6) kejadian per trip tahun. Terdapat lima basic event yang paling signifikan menurut besaran importance dari yang paling tinggi yaitu: Routine Violation–Mualim, Skill-based Error–Serang, Exceptional Violation–Nakhoda, Skill-based Error–Masinis, dan Skill-based Error–Juru Mudi. Kelima basic event tersebut dianalisis lebih lanjut terkait preconditions for unsafe acts. Hasil analisis didominasi oleh dua kategori, sebesar 60.47% dengan kategori coordination, communication, and planning, serta 30.23% dengan kategori fitness for duty.

The National Transportation Safety Committee (NTSC) recorded 147 shipping accidents between 2017 and 2023. Burning ship accidents dominated the types of accidents identified, with an incidence of 32%. According to statistics, more than 75% of marine accident fatalities are caused by human error. In this study, the author analyzed the crew factors in Ro-Ro ship fire cases in Indonesia. Qualitative and quantitative analysis was conducted using the Fault Tree Analysis (FTA) method to describe the failure of the crew's role and its impact on the failure in handling Ro-Ro ship fires. The results of the analysis show that the probability value of fire handling failure due to crew role failure is 1.02×10-6 events per trip year. There are five basic events that are most significant according to the importance measures from the highest, namely: Routine Violation–Ship Officer, Skill-based Error–Boatswain, Exceptional Violation–Master, Skill-based Error–Engineer, and Skill-based Error–Quarter Master. The five basic events were further analyzed related to preconditions for unsafe acts. The results of the analysis were dominated by two categories, 60.47% with the category of coordination, communication, and planning, and 30.23% with the category of fitness for duty.
