Abstract

Kebakaran pada kapal tanker LPG merupakan salah satu kejadian yang berbahaya dan berpotensi menyebabkan kerugian besar baik dari segi ekonomi maupun keselamatan jiwa. Penelitian ini bertujuan untuk menyelidiki penyebab kebakaran pada ruang holds space kapal tanker LPG akibat kebocoran gas. Penelitian ini dimulai dengan pengumpulan data insiden kebakaran dari berbagai sumber, termasuk laporan investigasi, wawancara dengan investigator, dan data operasional kapal. Data yang terkumpul dianalisis menggunakan Root Cause Analysis (RCA) untuk mengidentifikasi faktor-faktor penyebab utama, seperti kegagalan peralatan, kesalahan manusia, dan kondisi lingkungan. Rekomendasi dibuat dari berdasarkan RCA yang telah dibuat. Penelitian ini menggunakan metode Root Cause Analysis (RCA) untuk menganalisis penyebab kebakaran pada holds space kapal LPG Tanker. Model 4M Overturned Pyramid (MOP) diadopsi pada RCA sebagai pendekatan epidemiologi untuk karakterisasi Marine Traffic System (MTS). Hasil dari penelitian ini, ditemukan bahwa faktor utama penyebab kebakaran adalah kesalahan manusia. Tidak adanya standard operating procedures (SOP) dan kurangnya kemampuan mendeteksi risiko tinggi merupakan faktor yang paling berpengaruh dalam kasus ini. Berdasarkan hasil RCA dari penelitian ini, disarankan beberapa tindakan pencegahan untuk mengurangi risiko kebakaran di masa mendatang. Rekomendasi yang diberikan adalah pembuatan SOP untuk pekerjaan berisiko tinggi dan pelatihan untuk awak kapal.

Fires on LPG tankers are a dangerous event and have the potential to cause major losses both in terms of economics and life safety. This research aims to investigate the causes of fires in the holds space of LPG tankers due to gas leaks. This research began with collecting fire incident data from various sources, including investigation reports, interviews with investigators, and ship operational data. The collected data is analyzed using Root Cause Analysis (RCA) to identify the main causal factors, such as equipment failure, human error, and environmental conditions. Recommendations are made based on the RCA that has been made. This research uses the Root Cause Analysis (RCA) method to analyze the causes of fires in the holds space of LPG Tanker ships. The 4M Overturned Pyramid (MOP) model was adopted in RCA as an epidemiological approach for characterization of the Marine Traffic System (MTS). The results of this research found that the main factor causing fires was human error. The absence of standard operating procedures (SOP) and the lack of ability to detect high risks are the most influential factors in this case. Based on the RCA results from this study, several preventative measures are recommended to reduce the risk of future fires. The recommendations given are the creation of SOPs for high-risk work and training for ship crew.
