Abstract

The condensate transfer process using a jetty poses high risks to safety, the environment, and operational efficiency. This study aims to identify failure modes, evaluate risks using the Failure Mode, Effects, and Criticality Analysis (FMECA) method, and propose mitigation recommendations based on risk prioritization. Five main failure modes were identified: pipe leakage, flexible hose leakage, flexible hose misinstallation, adverse weather conditions, and suboptimal pump pressure. The analysis revealed that pipe leakage has the highest Risk Priority Number (RPN) of 36,40, making it the top mitigation priority. Recommended actions include routine pipe inspections, periodic flexible hose replacements, operator training, and the adoption of detection technology. The findings were validated through interviews with expert respondents, indicating that the recommendations are relevant and applicable. This study is expected to enhance risk management in condensate transfer operations using a jetty.
