Abstract
Limbah cair yang dihasilkan oleh RSUD Dr. R. Koesma Tuban memiliki karakteristik berbahaya dan dapat mencemari lingkungan jika tidak dikelola dengan baik. Dengan meningkatnya volume limbah akibat bertambahnya fasilitas kesehatan, risiko kegagalan dalam pengolahan air limbah juga meningkat. Oleh karena itu, penting untuk melakukan analisis risiko yang sistematis untuk memastikan bahwa proses pengolahan limbah berjalan dengan efektif dan memenuhi standar baku mutu yang ditetapkan. Metode yang digunakan dalam penelitian ini adalah Failure Mode and Effect Analysis (FMEA). FMEA diterapkan untuk mengidentifikasi berbagai mode kegagalan yang mungkin terjadi dalam sistem IPAL, melalui wawancara dan pengisian kuisioner dengan pengelola serta operator. Setiap mode kegagalan dinilai berdasarkan severity, occurrence, dan detection, yang kemudian dihitung untuk mendapatkan nilai Risk Priority Number (RPN). Setelah itu, dilakukan analisis penyebab kegagalan utama. Dari hasil analisis yang didapatkan dilakukan perencanaan tindakan preventif pada masing – masing penyebab kegagalan utama. Berdasarkan hasil identifikasi unit pengolahan IPAL, didapatkan unit pengolahan yang tidak bekerja dengan optimal sehingga perlu dilakukan analisis penyebab kegagalan. Berdasarkan identifikasi penyebab kegagalan dengan metode FMEA, didapatkan empat kelompok penyebab yakni manajerial, metode, lingkungan, dan unit pengolahan. Hasil analisis tersebut, didapatkan nilai RPN hasil perkalian severity, occurrence, dan detection. Nilai RPN ertinggi yang didapatkan adalah 125 yaitu debit influen, waktu detensi bak pengendap, dan beban permukaan (OFR) bak pengendap. Penyebab potensial dengan nilai RPN 80 yaitu pelatihan terkait IPAL dan penggantian media filter. Penyebab kegagalan dengan nilai RPN tertinggi, dilakukan analisis penyebab kegagalan utama. Berdasarkan hasil analisis, diberikan rekomendasi tindakan preventif yang dapat diimplementasikan untuk meningkatkan kinerja IPAL dan mengurangi risiko kegagaan dalam pengolahan limbah cair di IPAL RSUD Dr. R. Koesma Tuban.

The liquid waste generated by RSUD Dr. R. Koesma Tuban has hazardous characteristics and can contaminate the environment if not managed properly. With the increasing volume of waste due to the expansion of healthcare facilities, the risk of failure in wastewater treatment also rises. Therefore, it is essential to conduct a systematic risk analysis to ensure that the waste treatment process operates effectively and meets the established quality standards. The methods used in this study are Failure Mode and Effect Analysis (FMEA). FMEA is applied to identify various potential failure modes within the wastewater treatment system through interviews and discussions with managers and operators. Each failure mode is assessed based on severity, occurrence, and detection, which are then calculated to obtain the Risk Priority Number (RPN). Subsequently, an analysis of the root causes of the main failures is performed. Based on the analysis results, preventive action plans are developed for each oot cause of failure. Based on the identification results of the wastewater treatment units, it was found that some units were not operating optimally, necessitating an analysis of failure causes. The FMEA method identified four groups of potential causes: managerial, methodological, environmental, and operational units. The analysis yielded RPN values calculated from severity, occurrence, and detection. The highest RPN value obtained was 125, associated with influent flow rate, detention time in the sedimentation tank, and the overflow rate (OFR) of the sedimentation tank. Potential causes with RPN values between 80 included training related to WWTP and filter media replacement. For the failure causes with the highest RPN values, an analysis of the root causes was conducted. Based on these findings, recommendations for preventive actions will be provided to enhance the performance of the wastewater treatment plant and reduce the risk of failure in liquid waste treatment at RSUD Dr. R.Koesma Tuban.
