Abstract
Pekerjaan penggantian pipa gas termasuk dalam kategori pekerjaan berisiko tinggi karena memiliki potensi bahaya seperti kebocoran gas, ledakan, dan paparan gas beracun. PT PHR Limau field direkomendasikan untuk melakukan penggantian pipa gas trunkline sepanjang 9.4 km dengan ukuran 10 inch. Manajemen risiko menjadi penting pada pekerjaan ini, rendahnya penerapan analisis risiko dapat menjadi penghambat dalam eksekusi pekerjaan. Penelitian ini bertujuan untuk mengidentifikasi, menganalisis, dan membuat mitigasi risiko pada pekerjaan penggantian pipa trunkline menggunakan metode Failure Mode and Effects Analysis (FMEA) dan Bowtie Analysis. Hasil dari penelitian diperoleh sebanyak 3 variabel kategori risiko tinggi, 10 variabel risiko sedang, dan 5 variabel risiko rendah, risiko tersebut yaitu risiko terbakar saat pekerjaan pengelasan penyambungan antar pipa dengan risk ID (6a) dengan nilai RPN 80 dan pekerjaan pengelasan flange, risk ID (9a) dengan nilai RPN 80, serta risiko pekerjaan penggalian untuk lowering pipa dengan alat berat excavator, risk ID (8a) degan nilai RPN 75. Metode bowtie analisis digunakan untuk menentukan strategi tindakan preventif dan recovery dari 3 risiko utama tersebut. Risiko degan risk ID 6a dan 9a diperoleh faktor penyebabnya adalah loss of primary containment, adanya heat sources dan malfunction welding equipment. Dampak dari risiko tersebut adalah pekerja mengalami injury, fatality, asset damage dan enviromental pollution. Sedangkan risko dengan ID 8a penyebab dari risiko terkena swing excavator adalah operator fatique, dan blind spot area. Dampak dari risiko tersebut adalah pekerja mengalami injury, severe injury dan fatality. Perhitungan cost benefit anaylisis proyek dilakukan dengan asumsi pemenuhan aspek mitigasi risiko, diperoleh nilai cost benefit ratio (CBR) proyek 1.61 NPV positif dan IRR 21%, berdasarkan nilai tersebut proyek ini dianggap masih layak dan menguntungkan untuk dikerjakan. Penelitian ini dapat menjadi referensi bagi PT PHR untuk meningkatkan analisis risiko, mengurangi kerugian dan meningkatkan profit perusahaan.

The replacement of gas pipelines is classified as high-risk work due to the potential hazards, including gas leaks, explosions, and exposure to toxic gases. PT PHR Limau Field is recommended to replace a 9.4 km-long, 10-inch diameter gas trunkline pipeline. Effective risk management is crucial for this project, as insufficient risk analysis could hinder its successful execution. This study aims to identify, analyze, and develop mitigation strategies for risks associated with the trunkline replacement using the Failure Mode and Effects Analysis (FMEA) and Bowtie Analysis methods. The study identified 3 high-risk variables, 10 medium-risk variables, and 5 low-risk variables. The high-risk variables include: (1) the risk of fire during welding activities for pipe connections (Risk ID 6a) with an RPN value of 80; (2) the risk associated with welding flanges (Risk ID 9a) with an RPN value of 80; and (3) the risk of excavation work for pipe lowering using an excavator (Risk ID 8a) with an RPN value of 75. The Bowtie Analysis method was applied to develop preventive and recovery strategies for these three primary risks. Risks with Risk IDs 6a and 9a, the primary causes are the loss of primary containment, the presence of heat sources, and malfunctioning welding equipment. The potential impacts include worker injuries, fatalities, asset damage, and environmental pollution. Meanwhile, for Risk ID 8a, the risk of being struck by a swinging excavator is caused by operator fatigue and blind spots, with potential impacts including worker injuries, severe injuries, and fatalities. A cost-benefit analysis (CBA) of the project was conducted under the assumption that risk mitigation measures would be implemented. The analysis yielded a cost-benefit ratio (CBR) of 1.61, a positive Net Present Value (NPV), and an Internal Rate of Return (IRR) of 21%. Based on these figures, the project is deemed both feasible and profitable. This study provides a valuable reference for PT PHR to enhance risk analysis, reduce losses, and improve overall profitability
