Abstract
CV Wana Indo Raya merupakan perusahaan yang bergerak di bidang industri pengolahan kayu, khususnya produksi barecore. Aktivitas operasional dalam proses produksi barecore memiliki kompleksitas tinggi dan berpotensi menimbulkan berbagai risiko operasional yang dapat mengganggu efisiensi, keselamatan kerja, dan kualitas produk. Oleh karena itu, penelitian ini dilakukan untuk mengidentifikasi, menganalisis, mengevaluasi, dan memberikan rekomendasi penanganan risiko pada proses produksi barecore. Penelitian ini menggunakan pendekatan Fault Tree Analysis (FTA) untuk menelusuri akar penyebab risiko dan Failure Mode, Effect, and Criticality Analysis (FMECA) untuk mengevaluasi tingkat risiko berdasarkan tiga dimensi penilaian, yaitu severity, occurrence, dan detection. Hasil identifikasi menunjukkan bahwa terdapat 49 kejadian risiko yang tersebar pada tiga tahapan aktivitas utama, yaitu 11 risiko pada tahap inbound, 32 risiko pada proses produksi, dan 6 risiko pada tahap outbound. Hasil evaluasi risiko berdasarkan nilai Risk Priority Number (RPN) menunjukkan bahwa 41% dari total risiko tergolong dalam kategori extreme, 31% high, 24% medium, dan 4% low. Risiko prioritas ditentukan berdasarkan prinsip pareto dan diberikan rekomendasi penanganan menggunakan strategi avoiding risk, mitigating risk, dan transferring risk yang disusun sesuai penyebab utama pada lima kategori: Man, Machine, Method, Material, dan Environment. Rekomendasi ini tidak hanya difokuskan pada penurunan nilai RPN, namun juga sebagai pijakan awal dalam membangun budaya risk awareness dan sistem manajemen risiko yang sistematis, partisipatif, serta mendukung peningkatan kinerja operasional secara berkelanjutan.

CV Wana Indo Raya is a company operating in the wood processing industry, with a core focus on the production of barecore panels. Given the complexity and interdependence of processes within barecore manufacturing, various operational risks may emerge, potentially affecting product quality, production continuity, and occupational safety. This research aims to systematically identify, assess, and provide risk mitigation strategies for operational risks occurring in the barecore production process. The study employs a hybrid methodological approach, combining Fault Tree Analysis (FTA) for root cause identification with Failure Mode, Effect, and Criticality Analysis (FMECA) for evaluating the severity, occurrence, and detectability of each risk. The analysis identified 49 valid risks distributed across three stages of the production flow: inbound logistics (11 risks), core production (32 risks), and outbound processes (6 risks). Based on RPN calculations, 41% of the risks were categorized as extreme, 31% as high, 24% as medium, and 4% as low. Risk prioritization was conducted using the Pareto principle, followed by the formulation of recommended response strategies. These strategies comprise avoiding, mitigating, and transferring risks, with each recommendation designed in alignment with the five causal categories: Man, Machine, Method, Material, and Environment. Beyond numerical risk reduction, the proposed recommendations are intended as a strategic foundation to foster enterprise-wide risk awareness and support the development of a proactive and systematic risk management culture. The integration of these recommendations into operational practices is expected to enhance production resilience, process efficiency, and organizational adaptability.
