Abstract
Luka kronis adalah luka yang tidak melewati fase penyembuhan secara normal dan membutuhkan waktu lama. Gagalnya penutupan luka disebabkan oleh penyakit bawaan pasien (diabetes), tingginya konsentrasi reactive oxygen species dan infeksi mikroba. Penyembuhan luka kronis dapat ketika reactive oxygen species luka kronis turun menjadi luka akut. Proses penyembuhan ditandai dengan pengurangan inflamasi pada area luka. Upaya penyembuhan luka dapat dilakukan dengan metode debridement dan wound dressing. Pemberian wound dressing hydrogel enzimatik dengan sistem regenerasi secara in vitro mampu mengubah lingkungan luka kronis menjadi luka akut. Penelitian ini bertujuan untuk menganalisis respon penyembuhan luka kronis pada mencit Mus musculus diabetik terhadap pemberian wound dressing berbasis sistem regenerasi hidrogel-enzimatik (WDE). Respon yang diamati dari pemberian wound dressing tersebut berupa tingkat penyembuhan luka, kelimpahan bakteri, dan histologi. Penelitian ini memiliki tujuan melihat pengaruh pemberian wound dressing dengan sistem regenerasi terhadap kecepatan serta melihat secara seluler penyembuhan pada luka kronis Hasil penelitian menunjukkan kelompok WDE mencapai tingkat penyembuhan tertinggi (78,52%) namun pemberian wound dressing tidak berpengaruh terhadap penyembuhan secara statistika. Perlakuan WDE juga mampu menekan kelimpahan bakteri hingga 0 CFU/ml, Secara histologis, perlakuan WDE mendorong penyembuhan bersifat regeneratif, ditandai dengan re-epitelialisasi yang matang, jaringan granulasi yang padat, jumlah fibroblas yang signifikan, serta pembentukan jaringan adiposa baru. Sebaliknya, kelompok kontrol terjebak dalam fase inflamasi kronis dengan kerusakan jaringan. Dapat disimpulkan bahwa wound dressing WDE dengan sistem regeneratif mampu mempercepat penyembuhan luka kronis diabetes. Keberhasilan ini dicapai melalui mekanisme ganda, yaitu supresi bakteri yang efektif dan perbaikan jaringan secara regeneratif, menjadikannya pendekatan terapi yang sangat menjanjikan. =========================================================================================================================================
Chronic wound is a wound that does not pass through the normal phases of healing and requires a long time. The failure of wound closure is caused by the patient's underlying disease (diabetes), high concentrations of reactive oxygen species, and microbial infection. The healing of chronic wounds can occur when the reactive oxygen species of the chronic wound are reduced, shifting the wound to an acute state. The healing process is characterized by a reduction of inflammation in the wound area. Wound healing efforts can be made using debridement and wound dressing methods. The application of an enzymatic hydrogel wound dressing with a regenerative system is able, in vitro, to change the chronic wound environment into an acute wound.This study aims to analyze the chronic wound healing response in diabetic Mus musculus mice to the application of a wound dressing based on a hydrogel-enzymatic with regenerative system (WDE). The observed responses from the wound dressing application consisted of the rate of wound healing, bacterial abundance, and histology. This research has the objective of observing the effect of the regenerative wound dressing on the speed of healing as well as observing the cellular healing in chronic wounds. The research results show that the WDE group achieved the highest healing rate (78.52%), but the application of the wound dressing had no statistical effect on the healing. The WDE treatment was also able to suppress bacterial abundance to 0 CFU/ml. Histologically, the WDE treatment promoted regenerative healing, characterized by mature re-epithelialization, dense granulation tissue, a significant number of fibroblasts, and the formation of new adipose tissue. Conversely, the control group was stuck in a chronic inflammatory phase with tissue damage. It can be concluded that the WDE wound dressing with a regenerative system is able to accelerate the healing of diabetic chronic wounds. This success is achieved through a dual mechanism, namely effective bacterial suppression and regenerative tissue repair, making it a very promising therapeutic approach.
