Abstract

Kanker merupakan salah satu penyakit dengan penderita terbanyak dan juga memiliki tingkat kematian tertinggi di dunia. Salah satu kanker yang sering terjadi yaitu kanker paru. Kanker paru merupakan penyakit kanker terbanyak kedua setelah kanker payudara. kanker paru merupakan kanker yang paling sering didiagnosis, sebanyak 2,5 juta kasus baru atau satu dari delapan kanker di seluruh dunia (12,4%). Di Indonesia kanker paru menempati urutan ketiga setelah kanker payudara dan kanker serviks. Faktor utama kanker paru yaitu merokok secara aktif, namun terdapat juga penderita kanker paru yang bukan perokok aktif. Kanker paru dapat diobati menggunakan metode radioterapi, dimana sel kanker akan dihancurkan menggunakan sinar radiasi. Salah satu teknik dalam radioterapi yang dapat menghancurkan sel kanker tersebut adalah IMRT atau Intensity-Modulated Radiation Therapy. IMRT memiliki dua teknik utama dalam distribusi dosis, yaitu Sliding Window dan Step-and-Shoot. Penelitian ini bertujuan untuk menganalisis dan membandingkan aspek dosimetri serta efek radiobiologi dari teknik IMRT Sliding Window dan Step-and-Shoot pada kanker paru kanan. Penelitian ini menggunakan metode eksperimen berbasis perencanaan TPS dan studi fantom. Distribusi dosis antara teknik IMRT Sliding Window dan Step-anad-Shoot pada kasus kanker paru kanan menunjukkan kesamaan dan mampu memnuhi clinical goals. Teknik IMRT Sliding Window lebih unggul sedikit dibandingkan teknik IMRT Step-and-Shoot. Nilai NTCP pada teknik Sliding Window lebih rendah yang menandakan potensi toksisitas yang lebih rendah terhadap jaringan sehat. Nilai TCP yang lebih unggul dalam mengontrol tumor adalah teknik IMRT Step-and-Shoot. Dapat dilihat bahwa nilai area gamma 99,7% yang artinya hampir semua titik di area evaluasi klinis lulus gamma. Terdapat pula nilai gamma total sebesar 97% yang artinya ada beberapa low-dose atau gagal, tapi secara keseluruhan masih sangat baik, karena klinisnya tetap ≥ 95%.

Cancer is one of the diseases with the most sufferers and also has the highest mortality rate in the world. One of the most common cancers is lung cancer. Lung cancer is the second most common cancer after breast cancer. Lung cancer is the most frequently diagnosed cancer, with 2.5 million new cases or one in eight cancers worldwide (12.4%). In Indonesia, lung cancer ranks third after breast cancer and cervical cancer. The main factor in lung cancer is active smoking, but there are also lung cancer sufferers who are not active smokers. Lung cancer can be treated using radiotherapy, where cancer cells are destroyed using radiation rays. One technique in radiotherapy that can destroy cancer cells is IMRT or Intensity-Modulated Radiation Therapy. IMRT has two main techniques in dose distribution: Sliding Window and Step-and-Shoot. This study aims to analyze and compare the dosimetry aspects and radiobiological effects of Sliding Window and Step-and-Shoot IMRT techniques on right lung cancer. This study uses experimental methods based on TPS planning and phantom studies. The dose distribution between the Sliding Window and Step-and-Shoot IMRT techniques in right lung cancer cases showed similarities and were able to meet clinical goals. The Sliding Window IMRT technique was slightly superior to the Step-and-Shoot IMRT technique. The NTCP value in the Sliding Window technique was lower, indicating a lower potential for toxicity to healthy tissue. The TCP value that was superior in controlling tumors was the Step-and-Shoot IMRT technique. It can be seen that the gamma area value was 99.7%, which means that almost all points in the clinical evaluation area passed gamma. There was also a total gamma value of 97%, which means there were some low-doses or failures, but overall it was still very good, because the clinical results remained ≥ 95%.
