Abstract

Digital transformation in the healthcare sector has become a strategic urgency in responding to the growing risk of non-communicable catastrophic diseases that threaten the productive generation. Digital health products such as wearable devices, m-health applications, and online platforms offer innovative solutions for disease monitoring and prevention. However, adoption levels in Indonesia remain varied, particularly among Generation X and Generation Y, who face high health risks but have different digital characteristics. This study aims to empirically examine the factors influencing the intention to use and actual adoption of digital health products. The research employed a quantitative approach by distributing online surveys to Gen X and Gen Y respondents in Indonesia between July and September 2025. Empirical testing using PLS-SEM analysis revealed that health information literacy and digital literacy serve as upstream factors that significantly shape both intention and actual adoption of digital health products. Health information literacy was found to enhance awareness, perceived disease threat, and motivational passion, though not all effects directly influenced intention. In contrast, intention was more strongly driven by perceived threat, performance expectancy, and effort expectancy. Digital literacy emerged as a crucial catalyst that strengthened confidence in benefits while reducing perceived barriers, thereby fostering intention and adoption. Moderation analysis showed that health habits reinforced the relationships between awareness, perceived threat, harmonious passion, and effort expectancy with intention to use. Furthermore, intention was confirmed as a direct predictor of actual adoption, while also being moderated by perceived value. The general implications of this study highlight that the success of healthcare digital transformation in Indonesia depends greatly on the synergy between literacy, technology, healthy habits, and policy support. Enhancing health and digital literacy must be established as a national priority to strengthen the foundation of adoption, while product design should emphasize ease of use, tangible benefits, and socio-emotional value. The distinct characteristics of Gen X and Gen Y necessitate differentiated strategies in education, communication, and application feature design. At the policy level, usability standards, clinical integration, privacy regulations, and value-based incentives are required to ensure sustainable use.
