Abstract
Alat kesehatan merupakan alat yang berperan sangat penting dalam menunjuang kesehatan. Terutama di masa pandemi COVID-19, membuat kesadaran masyarakat akan pentingnya gaya hidup sehat meningkat. Syringe merupakan salahsatu alat kesehatan yang kebutuhannya meningkat dengan sangat pesat terutama ketika program vaksinasi COVID-19 berlangsung. Proses operasional pembuatan syringe harus mengutamakan sterilisasi produk. Penggunaan syringe yang tidak steril akan berakibat fatal untuk pasien yang menggunakannya. Oleh karena itu, perlu adanya manajemen risiko yang tepat karena dampak dari kemungkinan risiko yang terjadi dan tidak segera ditangani tidak hanya menyebabkan kerugian untuk perusahaan, namun juga dapat membahayakan pasien atau tenaga kesehatan yang menggunakan syringe tersebut dalam proses pengobatan, vaksinasi, maupun imunisasi. PT. X merupakan salah satu perusahaan manufaktur yang berlokasi di Krian, Sidoarjo, Jawa Timur yang memproduksi puluhan jenis alat kesehatan. Syringe merupakan salah satu produk alat kesehatan yang diproduksi oleh PT. X. Saat ini, manajemen risiko yang dimiliki hanya pada produksi syringe. Oleh karena itu, perlu adanya manajemen risiko untuk proses source, make, dan deliver pada produk syringe tersebut. Bayesian network merupakan salahsatu metode kuantitatif dalam menilai risiko. Bayesian network menghubungkan antar risiko yang saling memiliki hubungan sebab-akibat satu sama lain sehingga dapat memberikan gambaran risiko yang kompleks serta ketidakpastian model yang setiap faktor risikonya saling berkaitan. Penelitian Tugas Akhir ini akan melakukan penilaian risiko terhadap proses operasional supply chain pada produk syringe menggunakan metode Bayesian network. Adapun proses identifikasi risiko dilakukan mengidentifikasi proses operasional untuk produk syringe yang dikelompokkan ke dalam proses source, make, dan deliver. Lalu, setelah risiko dianalisa menggunakan Bayesian network, risiko dievaluasi dengan melakukan ranking risiko berdasarkan tingkatan kritis risiko tersebut. Selanjutnya, dilakukan mitigasi risiko pada risiko kritis yang ditemukan dari penilaian risiko. Dari setiap proses operasional, ditemukan 71 potensi risiko dengan 43 risiko prior dan 28 risiko posterior. Lalu, hasil dari proses analisis dan evaluasi risiko menggunakan Bayesian Network, ditemukan terdapat 17 risiko kritis dengan mayoritas risiko terdapat pada proses operasional make khususnya sterilisasi. Lalu, dilakukan rancangan mitigasi risiko pada risiko kritis tersebut sehingga menurunnya probabilitas potensi risiko prior dan posterior dan jumlah risiko kritis berkurang menjadi 6 potensi risiko.

Medical devices are tools that play a very important role in supporting health. Especially during the COVID-19 pandemic, increasing public awareness of the importance of a healthy lifestyle. Syringe is one of the medical devices whose needs are increasing very rapidly, especially during the COVID-19 vaccination program. The operational process of making syringes must prioritize product sterilization. The use of an unsterile syringe will be fatal for the patient who uses it. Therefore, it is necessary to have proper risk management because the impact of possible risks that occur and are not immediately handled will not only cause harm to the company, but can also endanger patients or health workers who use the syringe in the process of treatment, vaccination, or immunization. PT. X is a manufacturing company located in Krian, Sidoarjo, East Java, which produces dozens of types of medical devices. Syringe is one of the medical device products produced by PT. X. Currently, the only risk management is in the production of syringes. Therefore, there is a need for risk management for the process of source, make, and deliver the syringe product. Bayesian network is a quantitative method in assessing risk. The Bayesian network connects risks that have causal relationships with each other so that they can provide a complex risk picture and model uncertainty where each risk factor is interrelated. This final project research will carry out a risk assessment of the supply chain operational process on a syringe product using the Bayesian network method. The risk identification process is carried out to identify the operational processes for syringe products which are grouped into source, make, and deliver processes. Then, after the risk is analyzed using the Bayesian network, the risk is evaluated by ranking the risk based on the critical level of the risk. Furthermore, risk mitigation is carried out on the critical risks found from the risk assessment. From each operational process, 71 potential risks were found with 43 prior risks and 28 posterior risks. Then, the results of the risk analysis and evaluation process using the Bayesian Network, it was found that there were 17 critical risks with the majority of the risks contained in the operational process of makes, especially sterilization. Then, a risk mitigation design is carried out on the critical risk so that the probability of prior and posterior risks is reduced and the number of critical risks is reduced to 6 potential risks.
