Abstract
Penurunan fungsi ginjal secara progresif dan irreversibel yang berlangsung lebih dari tiga bulan dan disebabkan oleh adanya kelainan struktur ginjal disebut penyakit ginjal kronis. Penyakit ginjal kronis ditandai dengan laju filtrasi glomerulus (LFG) kurang dari 60 mL/menit/1,73 m2. Hasil Riset Kesehatan Dasar (Riskesdas) tahun 2018 oleh Badan Penelitian dan Pengembangan Kesehatan menunjukkan bahwa prevalensi penyakit ginjal kronis di Indonesia menunjukkan peningkatan dari tahun 2013, yaitu sebesar 0,18%. Bahkan, penyakit ginjal menjadi penyebab kematian ke-sepuluh terbanyak di Indonesia dengan jumlah kematian lebih dari 42 ribu per tahun. Penelitian ini bertujuan menganalisis faktor-faktor yang memengaruhi prevalensi penyakit ginjal kronis di Indonesia tahun 2023 menggunakan pendekatan regresi nonparametrik spline truncated. Unit analisis penelitian berupa 34 provinsi di Indonesia, dengan variabel prediktor meliputi rasio tenaga medis per 10.000 penduduk, rata-rata lama sekolah, Produk Domestik Regional Bruto (PDRB) per kapita, dan indeks kualitas air. Hasil analisis menunjukkan bahwa prevalensi penyakit ginjal kronis menurun seiring bertambahnya rasio tenaga medis dan PDRB per kapita serta meningkat seiring bertambahnya rata-rata lama sekolah dan meningkat seiring bertambahnya indeks kualitas air. Temuan ini memberikan dasar bagi pemerintah untuk meningkatkan distribusi tenaga medis, melaksanakan program edukasi kesehatan, memperbaiki kualitas air, dan mendorong kesejahteraan ekonomi, serta menjadi referensi bagi penelitian selanjutnya untuk mempertimbangkan prevalensi penyakit ginjal kronis dari waktu ke waktu untuk memahami pola jangka panjang dan faktor-faktor yang memengaruhinya.

Progressive and irreversible decline in kidney function lasting more than three months and caused by structural abnormalities in the kidneys is called chronic kidney disease. Chronic kidney disease is characterized by a glomerular filtration rate (GFR) of less than 60 mL/minute/1.73 m2. The results of the 2018 Basic Health Research (Riskesdas) by the Health Research and Development Agency show that the prevalence of chronic kidney disease in Indonesia has increased from 2013, which was 0.18%. In fact, kidney disease is the tenth leading cause of death in Indonesia, with more than 42,000 deaths per year. This study aims to analyze the factors influencing the prevalence of chronic kidney disease in Indonesia in 2023 using a truncated spline nonparametric regression approach. The research analysis unit consisted of 34 provinces in Indonesia, with predictor variables including the ratio of medical personnel per 10,000 population, average length of schooling, gross regional domestic product (GRDP) per capita, and water quality index. The results of the analysis show that the prevalence of chronic kidney disease decreases with an increase in the ratio of medical personnel and GRDP per capita, increases with an increase in average length of schooling, and increases with an increase in water quality index. These findings provide a basis for the government to improve the distribution of medical personnel, implement health education programs, improve water quality, and promote economic welfare, as well as serve as a reference for further research to consider the prevalence of chronic kidney disease over time to understand long-term patterns and influencing factors.
