Abstract
Penelitian ini dilatarbelakangi oleh kondisi geografis Kabupaten Kutai Barat yang didominasi oleh wilayah pedalaman dan lintasan Daerah Aliran Sungai (DAS) Mahakam, di mana aksesibilitas layanan kesehatan darat masih sangat terbatas. Saat ini, hanya terdapat lima fasilitas kesehatan tingkat pertama yang tersedia untuk melayani kebutuhan medis di 21 desa, sehingga memicu risiko tinggi terjadinya keterlambatan penanganan medis. Sebagai solusi, penelitian ini merencanakan pengembangan layanan kesehatan terapung dengan menerapkan metode Set Covering Problem (SCP) untuk menentukan lokasi pusat pelayanan yang paling optimal berdasarkan radius jangkauan kapal. Analisis mendalam dilakukan melalui dua skenario operasional yang berbeda: Skenario LCT mengutamakan efisiensi jumlah armada dengan penggunaan kapal Landing Craft Tank (LCT) sebagai Puskesmas terapung, sedangkan Skenario RST & kapal Ambulans berorientasi pada perluasan cakupan dan kontinuitas layanan melalui kombinasi operasional antara tongkang rumah sakit terapung dan kapal ambulans air. Skenario terpilih pada penelitian ini adalah LCT buat baru (1) unit, kapal tongkang bekas sebagai Rumah Sakit Terapung (3) unit, dan kapal ambulans bekas (15 ) unit, yang dinilai paling efisien secara teknis dan ekonomis. Dengan konfigurasi tersebut, sistem pelayanan kesehatan terapung mampu melayani sebanyak 218.178 pasien per tahun, dengan biaya pelayanan sebesar Rp1,105 juta per pasien dan biaya operasional harian sebesar Rp1,988 miliar. Hasil ini menunjukkan bahwa kombinasi pengadaan armada baru dan bekas dapat memberikan kinerja pelayanan yang optimal sekaligus menekan beban biaya, sehingga layak direkomendasikan sebagai solusi pelayanan kesehatan di wilayah perairan dan daerah terpencil.

This study is motivated by the geographical conditions of West Kutai Regency, which are dominated by inland areas and the Mahakam River Basin (DAS), where accessibility to land-based healthcare services remains highly limited. Currently, only five primary healthcare facilities are available to serve 21 villages, resulting in a high risk of delayed medical treatment. To address this issue, this study proposes the development of a floating healthcare service system by applying the Set Covering Problem (SCP) method to determine the optimal locations of service centers based on vessel coverage radius. A comprehensive analysis is conducted through two operational scenarios: the LCT scenario prioritizes fleet efficiency by utilizing Landing Craft Tank (LCT) vessels as floating primary healthcare centers, while the RST and ambulance vessel scenario focuses on expanding service coverage and continuity through an integrated operation of floating hospital barges and water ambulances. The selected scenario consists of one newly built LCT unit, three second-hand hospital barges as Floating Hospitals, and fifteen second-hand ambulance vessels, which is considered the most technically and economically efficient configuration. Under this configuration, the floating healthcare system is capable of serving 218,178 patients per year, with a service cost of IDR 1.105 million per patient and a daily operational cost of IDR 1.988 billion. These results indicate that a combination of newly built and second-hand vessels can deliver optimal healthcare service performance while minimizing overall costs, making it a feasible and effective solution for healthcare delivery in riverine and remote areas.
