Abstract
Permintaan customized alat berat di PT ABC mengindikasikan bahwa ada proses modifikasi desain yang menyebabkan proses produksi berbeda dari desain sebelumnya. Namun perubahan desain alat berat yang dilakukan, menyebabkan berbagai risiko muncul pada manajemen rantai pasok di PT ABC dan tidak bisa dipastikan, meskipun perubahan desain minor sekalipun. Sering sekali dampak dari perubahan desain ini bisa diidentifikasi saat proses produksi sudah berjalan, sehingga berdampak juga pada penundaan proses produksi. Tingkat pencapaian produksi di tahun 2020 sebesar 74,58% dan di tahun 2021 (akhir Agustus) sebesar 65,13% dari 100% yang direncanakan di awal tahun, artinya selisih dari persentase tersebut menunjukkan adanya penundaan produksi yang berdampak pada keterlambatan pengiriman produk ke pelanggan. Metodologi fuzzy FMEA digunakan untuk menganalisis potensi faktor-faktor penyebab keterlambatan pengiriman produk dan tingkat kepentingannya, di dalam manajemen rantai pasok produksi customized alat berat. Serta merumuskan rekomendasi untuk mencegah risiko keterlambatan pengiriman produk customized alat berat. Di dalam proses analisis nilai severity, occurance, dan detection diperoleh lewat focus group discussion dengan para ahli di PT ABC. Dari hasil penelitian dianalisis terdapat 68 risiko potensi faktor yang menyebabkan keterlambatan pengiriman produk, dan 11 risiko merupakan potensi faktor yang paling memberikan dampak keterlambatan pengiriman, dengan nilai fuzzy RPN tertinggi yaitu, 7,72 dari 2 potensi faktor, fuzzy RPN 9,25 dari 7 potensi faktor, dan 2 potensi faktor dengan nilai fuzzy RPN 9,37. Gambar produk yang tidak lengkap dan BOM yang tidak lengkap merupakan 2 faktor yang paling mempengaruhi manajemen rantai pasok di produksi customized alat berat. Berdasarkan hasil tingkat kepentingan potensi faktor keterlambatan, melalui FGD dihasilkan 11 rekomendasi perbaikan untuk 5 area department, yaitu department desain, department proses engineering, department purchasing, department pergudangan, dan department produksi.

Demand of customized heavy equipment from customer at PT ABC indicated there are modification process from the original design, that impact to the production process will different. The modification of heavy equipment will impact to the several risk in the supply chain management at PT ABC, and it unpredictable or uncertain, even if the modification of design are minor. The modification impact are often can be identified, when the production process already started, it will be impacted to delays in the production process. The production achievement on 2020 are 74,58% and on 2021 (end August) are 65,13% from 100% that already planned in the beginning of year, it’s mean the difference from the percentage are delay product delivery to the customers. The methodology Fuzzy FMEA are used to analyze the factors causing delay delivery and their importance (rank) in the supply chain management customized heavy equipment production. And also formulating the recommendation as risk mitigation of delay delivery. In the analysis process, the value of severity (S), occurance (O), and detection (D) obtained from focus group discussion with experts at PT ABC. From the results of the study, can be analyzed there are 68 potentials risk factors that cause delay delivery and there are 11 risks potential factors, that have most impact on delay delivery, with the highest fuzzy RPN value, 7,72 from 2 potential factors, 9,25 of fuzzy RPN from 7 potential factors, and 2 potential factors with highest value fuzy RPN are 9,37. The drawing completeness and the bill of material completeness, are the most factor that influnces supply chain management in customized heavy equipment production. Based on the rank of potential factors through FGD, 11 recommendations are formulated for improvement on 5 departments, design department, process engineering department, purchasing department, warehouse department, and production department.
