Abstract

Quality improvement in a bogie manufacturing industry has traditionally been carried out using several conventional quality tools applied sequentially. As a business organization, the company requires a comprehensive method that covers the identification of defect types to be prioritized through to the recommendation of improvement actions while taking cost into consideration. In previous research, the Quality Improvement Matrix (QIM) was developed as an integrated tool based on Failure Mode and Effect Analysis (FMEA) House of Quality (HOQ). The QIM, which consists of two matrices, can be used for comprehensive quality improvement, ranging from the identification of defects and their root causes to the prioritization of quality improvement actions. However, the QIM still has limitations: the assessment of Severity and Difficulty does not explicitly consider cost, and the Detection parameter is not taken into account. This research develops an enriched QIM framework through the integration of the Extended FMEA concept. The development covers three main aspects: (1) the addition of a Severity Cost parameter that accounts for the cost of failure, and a Detection parameter that measures the system's ability to detect the root causes of defects; (2) the enrichment of the Difficulty parameter with cost and implementation time aspects; and (3) the categorization of defect root causes based on the 6M. In addition, the quality improvement actions were modified by explicitly offering Jidoka as an optional method. The newly developed QIM was applied to help the company address defect problems in bogie production. The analysis identified 5 dominant defects and 32 root causes. The recommended priority order of quality improvement actions is the development of standard operating procedures, the provision of operator training, and the development of Poka-Yoke. Based on feedback from the company, the new QIM framework can assist the company in carrying out quality improvement. Furthermore, the presentation of the QIM analysis results in the form of two matrices is simple yet informative for communicating the quality improvement process flow to all relevant parties in the company.
