Abstract

Kekosongan stok obat di Instalasi Farmasi Rumah Sakit Krakatau Medika (RSKM) terjadi berulang dan berdampak langsung pada pasien: selama Januari–April 2025 tercatat 56 transaksi Emergency Purchase Order (EPO) dengan biaya 15–30% lebih mahal dari jalur reguler, serta 478 kejadian copy resep, yang bersumber bukan dari keterbatasan anggaran melainkan dari tidak adanya mekanisme yang mampu mendeteksi stok kritis sebelum benar-benar habis. Penelitian ini mendesain kembali proses pengendalian persediaan obat RSKM dari kondisi eksisting (as-is) menuju rancangan proses yang diusulkan (to-be), dengan memetakan alur pengadaan menggunakan SIPOC dan Value Stream Mapping (VSM), mengidentifikasi prioritas kegagalan proses melalui Failure Mode and Effects Analysis (FMEA), dan menghitung parameter pengendalian persediaan (ABC, Safety Stock, Reorder Point, dan Economic Order Quantity) berdasarkan lead time aktual per item — bukan asumsi seragam 14 hari yang selama ini digunakan. Hasil analisis mengidentifikasi dua titik kegagalan kritis: stok kritis yang tidak terdeteksi tepat waktu (RPN = 576) dan rencana kebutuhan obat (RKO) yang tidak mencerminkan konsumsi aktual (RPN = 392); koreksi lead time aktual (bervariasi 1–43 hari) menurunkan rata-rata Reorder Point sebesar 48–75% di tiga gudang. Berdasarkan temuan tersebut, dirancang proses to-be berupa sistem notifikasi tiga level yakni Yellow Alert saat stok menyentuh 30% stok maksimum, Orange Alert saat mencapai Reorder Point disertai rekomendasi kuantitas pemesanan EOQ, dan Red Alert saat stok turun di bawah Safety Stock dengan eskalasi otomatis ke manajemen yakni dilengkapi approval matrix lima tingkat yang selaras dengan struktur otorisasi keuangan RSKM dan SOP fail-safe 14 langkah. Perbaikan parameter RKO saja telah menurunkan EPO sebesar 44,6%; dengan implementasi penuh sistem notifikasi, proyeksi menunjukkan EPO dapat turun lebih dari 68% dan copy resep turun minimal 50%, melampaui target penelitian.

Recurring drug stockouts at the Pharmacy Installation of Krakatau Medika Hospital (RSKM) have direct consequences for patients: between January and April 2025 alone, 56 Emergency Purchase Order (EPO) transactions were recorded at costs 15–30% above regular procurement, alongside 478 copy-prescription incidents, stemming not from budget constraints but from the absence of any mechanism to detect critically low stock before it actually ran out. This study redesigns RSKM's inventory control process from its existing condition (as-is) toward a proposed process (to-be), mapping the procurement flow using SIPOC and Value Stream Mapping (VSM), prioritizing failure points through Failure Mode and Effects Analysis (FMEA), and calculating inventory control parameters (ABC classification, Safety Stock, Reorder Point, and Economic Order Quantity) using actual per-item lead times rather than the uniform 14-day assumption previously applied. The analysis identified two critical failure points: undetected critical stock conditions (RPN = 576) and a drug requirement plan (RKO) disconnected from actual consumption (RPN = 392); correcting lead times (ranging from 1–43 days) reduced average Reorder Points by 48–75% across three warehouses. Based on these findings, a to-be process was designed around a three-level notification system — Yellow Alert at 30% of maximum stock, Orange Alert at the Reorder Point with an EOQ-based order recommendation, and Red Alert below Safety Stock with automatic escalation to management — supported by a five-tier approval matrix aligned with RSKM's existing financial authorization structure and a 14-step fail-safe SOP. Correcting RKO parameters alone reduced EPO transactions by 44.6%; full implementation of the notification system is projected to reduce EPO by more than 68% and copy prescriptions by at least 50%, surpassing the study's targets.
