Abstract
Dysmenorrhea merupakan kondisi nyeri haid yang kerap disertai nyeri alih (referred pain) ke area punggung bawah. Terapi panas terbukti efektif sebagai intervensi non-farmakologis; namun, produk komersial saat ini memiliki keterbatasan berupa distribusi panas tunggal dan ketiadaan sistem keamanan perangkat keras (hardware) independen, sehingga memunculkan kerentanan terhadap risiko overheating dan luka bakar suhu rendah. Penelitian ini merupakan pilot study yang bertujuan merancang bangun dan mengevaluasi kelayakan awal prototipe sabuk penghangat dual-heater (area abdomen dan lumbal). Sistem dikendalikan oleh mikrokontroler Arduino Nano menggunakan algoritma Proportional-Integral-Derivative (PID) untuk mempertahankan stabilitas suhu terapeutik pada rentang 38–45°C. Sebagai mekanisme pengaman absolut (fail-safe), Thermal Fuse 185°C diintegrasikan secara seri guna memutus aliran listrik secara permanen saat terjadi anomali termal ekstrem. Hasil pengujian teknis mengonfirmasi bahwa sistem kendali PID beroperasi stabil dengan error validasi klinis rata-rata maksimal sebesar ±0,30°C dan mampu menekan lonjakan suhu (overshoot) secara signifikan. Evaluasi klinis terbatas pada 3 responden penderita dysmenorrhea menunjukkan bahwa intervensi terapi selama 20 menit menghasilkan penurunan intensitas nyeri dengan rata-rata selisih skor Numerical Rating Scale (NRS) sebesar 3,0 poin, di mana seluruh responden (3/3) berhasil melampaui ambang batas Minimum Clinically Important Difference (MCID). Evaluasi usabilitas menunjukkan tingkat penerimaan pengguna yang positif dengan rata-rata skor 3,14 dari skala 4,00. Prototipe ini terbukti fungsional, aman, dan berpotensi efektif, sehingga layak dikembangkan lebih lanjut pada uji klinis skala penuh.

Dysmenorrhea is a menstrual pain condition frequently accompanied by referred pain to the lower back. While heat therapy is a proven non-pharmacological intervention, current commercial products are limited by single-area heat distribution and the absence of an independent physical hardware safety system, exacerbating vulnerability to overheating and low-temperature burns. This pilot study aimed to design and evaluate the initial feasibility of a dual-heater heating belt prototype (abdomen and lumbar regions). The system is controlled by an Arduino Nano microcontroller utilizing a Proportional-Integral-Derivative (PID) algorithm to maintain therapeutic temperature stability within the 38–45°C range. As an absolute fail-safe mechanism, a 185°C Thermal Fuse is integrated in series to permanently disconnect the electrical current during extreme thermal anomalies. Technical evaluation results confirmed that the PID control system operated stably with a maximum average clinical validation error of ±0.30°C and successfully suppressed temperature overshoot significantly. A limited clinical evaluation of 3 respondents suffering from dysmenorrhea demonstrated that a 20-minute therapeutic intervention resulted in an average Numerical Rating Scale (NRS) pain reduction of 3.0 points, with all respondents (3/3) successfully achieving the Minimum Clinically Important Difference (MCID) threshold. Usability evaluation indicated positive user acceptance with an average score of 3.14 out of a 4.00 scale. This prototype is proven to be functional, safe, and potentially effective, making it feasible for further development in full-scale clinical trials.
