Abstract
Strok berulang merupakan permasalahan penting pada pasien pascastrok karena dapat meningkatkan risiko kecacatan dan kematian. Pasien strok dengan riwayat merokok memerlukan perhatian khusus karena memiliki paparan faktor risiko vaskular yang dapat berkontribusi terhadap terjadinya kekambuhan. Penelitian ini bertujuan untuk mendeskripsikan karakteristik pasien strok dengan riwayat merokok, menggambarkan pola rata-rata kumulatif kejadian berulang pertama, serta menganalisis faktor-faktor yang memengaruhi hazard kejadian berulang pertama strok. Data yang digunakan merupakan data sekunder rekam medis pasien rawat inap dengan diagnosis strok di RSUD Haji Provinsi Jawa Timur selama periode 1 Januari 2023 hingga 31 Desember 2025. Data awal terdiri atas 304 pasien. Setelah dua pasien dengan lebih dari satu kejadian berulang setelah baseline tidak diikutsertakan, analisis utama dilakukan terhadap 302 pasien, yaitu 272 pasien tersensor dan 30 pasien yang mengalami kejadian berulang pertama. Dalam struktur start-stop, 30 pasien yang mengalami kejadian berulang pertama ditambahkan sebagai interval lanjutan tersensor, sehingga data pemodelan terdiri atas 302 pasien unik dengan total 332 baris interval pengamatan. Analisis deskriptif dilakukan menggunakan Mean Cumulative Function (MCF), sedangkan pemodelan dilakukan menggunakan model Prentice-Williams-Peterson Total Time dengan pembatasan pada kejadian berulang pertama. Nilai akhir MCF sebesar 0,138 kejadian per pasien. Berdasarkan seleksi model menggunakan AIC dan AICc, model terbaik hanya memuat variabel diabetes melitus. Pasien dengan diabetes melitus memiliki hazard mengalami kejadian berulang pertama sebesar 3,0617 kali dibandingkan pasien tanpa diabetes melitus, dengan p-value sebesar 0,0022 dan interval kepercayaan 95% sebesar 1,4956 hingga 6,2679.

Recurrent stroke is an important clinical problem among post-stroke patients because it may increase the risk of disability and mortality. Stroke patients with a history of smoking require particular attention because they have exposure to vascular risk factors that may contribute to recurrence. This study aims to describe the characteristics of stroke patients with a history of smoking, illustrate the mean cumulative pattern of first recurrent stroke events, and analyze factors affecting the hazard of first recurrent stroke events. The data used in this study were secondary medical record data of hospitalized stroke patients at RSUD Haji Provinsi Jawa Timur from 1 January 2023 to 31 December 2025. The initial data consisted of 304 patients. After excluding two patients with more than one recurrent event after baseline, the main analysis included 302 patients, consisting of 272 censored patients and 30 patients who experienced a first recurrent stroke event. In the start-stop structure, the 30 patients who experienced a first recurrent stroke event were added as censored follow-up intervals, resulting in modeling data consisting of 302 unique patients with a total of 332 observation intervals. Descriptive analysis was performed using the Mean Cumulative Function (MCF), while modeling was conducted using the Prentice-Williams-Peterson Total Time model with the analysis restricted to the first recurrent event. The final MCF value was 0.138 events per patient. Based on model selection using AIC and AICc, the best model included only diabetes mellitus. Patients with diabetes mellitus had a hazard of experiencing a first recurrent stroke event 3.0617 times higher than patients without diabetes mellitus, with a p-value of 0.0022 and a 95% confidence interval of 1.4956 to 6.2679.
