Abstract

Tuberkulosis (TB) paru masih menjadi beban kesehatan masyarakat yang signifikan di Kota Surabaya dengan pola persebaran kasus antarwilayah yang tidak merata. Penelitian ini bertujuan mengembangkan sistem informasi surveilans TB paru berbasis web untuk mendukung pengelolaan data pasien, pemantauan pasien terlambat berkunjung, visualisasi persebaran kasus, penentuan prioritas tindak lanjut, dan estimasi perubahan kasus operasional pada tingkat kelurahan. Penelitian menggunakan data kasus TB dari tujuh puskesmas yang mencakup 28 kelurahan pada Juli 2025 hingga Maret 2026, disertai data kependudukan dan lingkungan. Sistem dikembangkan dengan arsitektur client-server menggunakan React.js, Node.js/Express.js, dan PostgreSQL/PostGIS. Analisis terhadap 252 observasi kelurahan-bulan menunjukkan seluruh variabel tidak menyebar normal dengan indikasi overdispersi, sehingga pencocokan distribusi menempatkan Lognormal sebagai distribusi terbaik. Uji beda rata-rata antarkecamatan menunjukkan perbedaan yang nyata. Keterkaitan variabel lingkungan diperiksa secara eksploratif melalui regresi Lognormal sebagai model utama dan regresi Binomial Negatif sebagai pembanding; suhu permukaan lahan, kelembapan relatif, dan kawasan terbangun belum menunjukkan hubungan yang nyata, sehingga digunakan sebagai konteks pemantauan. Sistem berhasil menyediakan dashboard, manajemen data pasien, pemantauan kunjungan tertunda, peta persebaran kasus, pelaporan, dan pengingat tindak lanjut. Evaluasi pengguna menunjukkan peningkatan indeks manfaat dari 48,96% menjadi 76,56%. Sistem ini mendukung surveilans TB paru yang lebih terstruktur, informatif, dan terintegrasi.

Pulmonary tuberculosis (TBC) remains a significant public health burden in Surabaya, with the spatial distribution of cases across areas serving as an important consideration in determining intervention priorities. This study aimed to develop a web-based pulmonary TB surveillance information system to support patient data management, delayed-visit monitoring, case distribution visualization, follow-up prioritization, and operational case-change estimation at the urban-village level. The study used TB case data from seven primary health centers covering 28 urban villages from July 2025 to March 2026, together with population and environmental data, including land surface temperature, relative humidity, built-up area percentage, and population density. The system was developed using a client–server architecture comprising React.js, Node.js/Express.js, and PostgreSQL/PostGIS. Analysis of 252 urban-village–month observations showed that all variables were non-normally distributed with indications of overdispersion; distribution fitting identified the lognormal distribution as the best fit. A one-way analysis of variance showed significant differences in mean case counts among districts. Associations of environmental variables were examined exploratorily using lognormal regression as the main model and negative binomial regression as a comparison; land surface temperature, relative humidity, and built-up area showed no significant associations and were therefore treated as monitoring context. The system integrated dashboards, patient management, treatment monitoring, case distribution maps, reporting, follow-up reminders, and role-based access control. User evaluation showed an increase in the perceived benefit index from 48.96% to 76.56%. Overall, the system supports more structured, integrated, and informative pulmonary TB surveillance.
