Abstract

Acute lymphoblastic leukemia (ALL) merupakan jenis leukemia yang paling sering terjadi pada anak dan memiliki risiko relapse selama masa pengobatan maupun tindak lanjut. Penelitian ini bertujuan untuk mendeskripsikan karakteristik pasien, menggambarkan event-free survival (EFS), serta menganalisis faktor-faktor yang memengaruhi EFS pada pasien ALL anak di RSUD Dr. Soetomo Surabaya menggunakan regresi Cox. Data yang digunakan merupakan data sekunder rekam medis sebanyak 144 pasien ALL anak. Analisis dilakukan menggunakan statistik deskriptif, kurva Kaplan-Meier, uji Log-rank, dan regresi Cox. Hasil penelitian menunjukkan bahwa probabilitas EFS pada bulan ke-12, ke-24, dan ke-36 berturut-turut sebesar 95,0%, 87,1%, dan 73,9%. Uji Log-rank menunjukkan tidak terdapat perbedaan EFS yang signifikan berdasarkan karakteristik leukosit pasien ALL anak. Hasil uji asumsi proportional hazards menunjukkan bahwa variabel usia melanggar asumsi, sehingga digunakan extended Cox regression dengan time-varying coefficient. Model akhir menggunakan regresi Cox extended menunjukkan bahwa pasien dengan jumlah leukosit lebih dari sama dengan 50.000 sel/ μL memiliki hazard relapse sebesar 2,814 kali dibandingkan pasien dengan jumlah leukosit kurang dari 50.000 sel/ μL. Selain itu, pengaruh usia terhadap hazard relapse berubah seiring bertambahnya waktu pengamatan. Hasil penelitian ini diharapkan dapat membantu tenaga kesehatan dalam mengidentifikasi pasien dengan risiko relapse yang lebih tinggi.

Acute lymphoblastic leukemia (ALL) is the most common type of leukemia in children and carries a risk of relapse during both treatment and follow-up. This study aimed to describe patient characteristics, report event-free survival (EFS), and analyze factors influencing EFS in pediatric ALL patients at Dr. Soetomo General Hospital in Surabaya using Cox regression. The data used consisted of secondary medical records from 144 patients. Analysis was performed using descriptive statistics, Kaplan-Meier curves, the Log-rank test, and Cox regression. The results showed that the probability of EFS at months 12, 24, and 36 was 95.0%, 87.1%, and 73.9%, respectively. The Log-rank test showed no significant differences in EFS based on the characteristics of the patients studied. The results of the proportional hazards assumption test indicated that the age variable violated the assumption; therefore, Extended Cox Regression with time-varying coefficients was used. The final model showed that patients with a white blood cell count of greater than or equal to 50.000 cells/ μL had a 2.814-fold higher relapse hazard compared to patients with a white blood cell count of less than 50.000 cells/ μL. Additionally, the effect of age on the relapse hazard changed as the observation period increased. The results of this study are expected to assist healthcare professionals in identifying patients at higher risk of relapse.
