Abstract

Stimulasi elektrik (electrical stimulation/ES) telah dipelajari untuk digunakan dalam terapi percepatan penyembuhan luka. Akan tetapi, pemakaian metode ini pada tingkat klinis masih sangat terbatas dan umumnya menggunakan sistem open-loop. Tujuan penelitian ini adalah membuat stimulasi elektrik closed-loop yang mampu menentukan durasi ES secara optimal sejalan dengan pengurangan luas luka. Sistem stimulasi elektrik penyembuhan luka ini tersusun dari modul rangkaian boost converter, generator pulsa, dan pemroses data berbasis mikrokontroler ATmega32. Parameter ES yang diatur adalah tegangan dan durasi ES, sedangkan frekuensi dan lebar pulsa dibuat tetap (20 Hz, $250\ \mu\text{s}$) dengan arus tidak lebih dari $60\ \mu\text{A}$. Oleh karena percepatan penyembuhan luka memiliki karakteristik nonlinier dan berubah pada setiap tahapnya, penelitian ini menguji pengendalian durasi ES menggunakan kontroler fuzzy. Kontroler fuzzy digunakan dalam sistem ES closed-loop karena keefektifannya dalam mengatasi respons penyembuhan luka yang dinamis dan kompleks. Realisasi kontroler fuzzy dilakukan dengan menentukan durasi ES berikutnya berdasarkan hasil deteksi pencapaian pengurangan luas luka. Pengukuran pengurangan luas luka menggunakan metode pengolahan citra berbasis informasi warna luka. Pengujian parameter menunjukkan kesalahan rata-rata frekuensi sebesar 2,5%, lebar pulsa 17%, durasi 1,6%, dan pengurangan luas luka 1,5%. Kemampuan kontroler dalam memberikan durasi ES diuji coba pada dua hewan coba marmut. Hasil penelitian menunjukkan bahwa percepatan penyembuhan luka dengan ES closed-loop 1,5 kali lebih cepat daripada ES open-loop dan 2 kali lebih cepat daripada hewan tanpa terapi ES. Dari hasil ini, sistem ES closed-loop dapat diaplikasikan secara klinis untuk mempercepat penyembuhan luka. Perlu dilakukan penelitian lanjutan dengan jumlah sampel yang lebih besar untuk mengeksplorasi parameter lain yang berpengaruh pada proses penyembuhan luka.

Electrical stimulation (ES) has been studied as a therapy for accelerating wound healing. However, the clinical application of this method remains very limited and typically relies on open-loop systems. The goal of this research is to develop a closed-loop electrical stimulation system that determines optimal stimulation duration based on the reduction of wound area. The electrical stimulation system for wound healing consists of a series of modules including a boost converter, a pulse generator, and microcontroller ATmega32-based data processing. Adjustable ES parameters include voltage and stimulation duration, while frequency and pulse width are fixed at 20 Hz and 250 μs, respectively, with a current not exceeding 60 μA. Because wound healing acceleration exhibits nonlinear characteristics and varies across phases, this study tests the control of ES duration using a fuzzy controller. The fuzzy controller is employed in the closed-loop ES system due to its effectiveness in managing dynamic and complex wound healing responses. The fuzzy controller determines the next ES duration based on the detected reduction in wound area. Wound area reduction is measured using an image processing method based on color detection. Experimental evaluations yield an average parameter error of 2.5% for frequency, 17% for pulse width, 1.6% for duration, and 1.5% for wound area reduction. The performance of the controller in regulating ES duration was tested on two guinea pigs. The results show that wound healing acceleration using closed-loop ES is 1.5 times faster than open-loop ES and 2 times faster than control subjects without ES therapy. These findings indicate that closed-loop ES for wound healing holds potential for clinical applications to accelerate healing. Further research should involve a larger sample size to identify additional parameters that influence the wound healing process.
