Abstract

Penelitian bertujuan memverifikasi distribusi dosis pada target antara perencanaan perawatan radioterapi dengan pengukuran film EBT3 dan membandingkan rencana perawatan IMRT dan VMAT berdasarkan analisis dengan parameter NTCP/TCP dan HI untuk kasus orofaring. Penelitian diawali dengan fabrikasi fantom berbahan dasar polylactic acid (PLA), kemudian dilakukan kalibrasi film GafChromic EBT3. Setelah didapatkan persamaan dari kurva kalibrasi, dilakukan verifikasi antara TPS dengan hasil pengukuran. Faktor gEUD dimasukkan pada TPS, kemudian dianalisis efek radiobiologi dan dosimetrinya. Nilai distribusi dosis hasil pengukuran dengan film GafChromic EBT3 dan simulasi TPS pada teknik perawatan Intensity Modulated Radiotherapy (IMRT) dan Volumetric Modulated Arc Therapy (VMAT) memiliki perbedaan dengan persentase untuk OAR mata didapatkan di bawah 200%, untuk kelenjar parotis didapatkan di bawah 30%, untuk spinal cord didapatkan di bawah 5%, dan untuk brainstem didapatkan di bawah 7%. Sedangkan pada PTV di bawah 1%. Probabilitas komplikasi jaringan normal berdasarkan parameter NTCP mendekati 0% untuk semua OAR. Probabilitas control tumor bernilai 100%. Homogenitas distribusi dosis hampir homogen karena bernilai mendekati nol.

The aim of this study was to verify the dose distribution in the target between radiotherapy treatment planning and EBT3 film measurements and to compare IMRT and VMAT treatment plans based on analyses using NTCP/TCP and HI parameters for oropharyngeal cases. The study began with the fabrication of a phantom made from polylactic acid (PLA), followed by the calibration of GafChromic EBT3 film. After obtaining the equation from the calibration curve, verification was carried out between the TPS and the measurements. The gEUD factor was incorporated into the TPS, followed by an analysis of its radiobiological effects and dosimetry. The dose distribution measured using GafChromic EBT3 film and the TPS simulation for the Intensity Modulated Radiotherapy (IMRT) and Volumetric Modulated Arc Therapy (VMAT) treatment techniques showed differences, with percentages for the eye OAR below 200%, the parotid gland below 30%, the spinal cord below 5%, and the brainstem below 7%. Meanwhile, the difference for the PTV was below 1%. The probability of normal tissue complications based on the NTCP parameters was close to 0% for all OARs. The probability of tumor control was 100%. The dose distribution was considered nearly homogeneous because the homogeneity index value was close to zero.
