Abstract

Indonesia adalah salah satu daerah endemik Malaria. Diagnosis yang tepat diperlukan untuk penanganan Malaria. Salah satu bagian terpenting dari diagnosis adalah deteksi parasit malaria terutama pada apusan tebal darah. Lokasi daerah endemik yang kebanyakan di daerah terpencil dengan sarana dan prasarana minim menyulitkan proses deteksi. Hal itu disebabkan karena sulitnya menyediakan citra mikroskopis apusan tebal darah yang berkualitas tinggi dibanding yang berkualitas rendah. Karakteristik dari apusan tebal darah kualitas rendah mempersulit deteksi parasit karena adanya noise yang tinggi, kemiripan intensitas antara background dan obyek, kemungkinan munculnya artefak, dan citra yang kabur. Selain itu, kurangnya microscopist yang dapat melakukan pembacaan apusan darah, menyebabkan deteksi parasit malaria menjadi lambat. Untuk itu pada penelitian ini akan dibangun sistem otomasi deteksi parasit malaria dari citra mikroskopis apusan tebal darah kualitas rendah yang diperoleh dari daerah endemik malaria di Indonesia.
Metode yang diusulkan terdiri dari tiga tahapan utama. Pertama, preprocessing pada citra apusan tebal darah kualitas rendah karena citra ini sulit terbaca keberadaan parasitnya. Pada tahap ini dilakukan image enhancement menggunakan edge correction berbasis pada dual contrast enhancement untuk meningkatkan kontras dan mempertajam garis obyek. Kedua, segmentasi dan klasifikasi komponen darah untuk mendapatkan kandidat parasit dari citra apusan tebal darah menggunakan intensity slicing, operasi morfologi, dan rule berbasis fitur dari inti parasit. Ketiga, identifikasi kandidat parasit yang dapat menurunkan false positif pada penentuan status parasit malaria. Tahap ini terdiri dari proses segmentasi menggunakan Watershed, Index of Fuzziness, dan Adaptive Thresholding serta pemanfaatan fitur parasit untuk menentukan statusnya Kinerja dievaluasi menggunakan citra mikroskopis apusan tebal darah dari Lembaga Biologi Molekuler Eikjman. Hasil penelitian menunjukkan bahwa 1) metode enhancement yang diusulkan mencapai rerata entropy tertinggi yaitu 7.03 dengan peningkatan sebesar 23% dibandingkan citra sebelum enhancement. Hal ini berarti bahwa metode usulan dapat memproduksi citra dengan kandungan informasi lebih baik dibandingkan metode yang lain; 2) metode usulan untuk segmentasi kandidat parasit mampu memperoleh rerata presisi, recall, and F-measure sebesar 86%. Penggunaan fitur inti parasit dalam klasifikasi komponen darah terbukti dapat meningkatkan presisi sistem; 3) metode identifikasi kandidat parasit dengan memanfaatkan fitur dari parasit mampu menurunkan false positive lebih besar dari 70% dibandingkan hasil sistem pada tahap segmentasi dan klasifikasi komponen darah. Berbasis hasil-hasil tersebut bisa disimpulkan bahwa metode usulan tepat digunakan pada proses deteksi parasit malaria.

Indonesia is one of the malaria endemic areas. Proper diagnosis is needed for the Malaria handling. An important step of the diagnosis is the detection of malaria parasites, especially on the thick blood smear. Location endemic areas that mostly in remote areas with poor infrastructure complicate the detection process. It is caused due to the difficulty of providing a thick blood smear microscopic image with standard quality instead of the low quality. Characteristics of thick blood smear with low quality complicate the parasite detection due to high noise, the similar intensity between the background and the object, the possibility of artifacts, and blurred image. In addition, the lack of microscopist who can read the blood smear, causing the detection of malaria parasites to be slow. Therefore in this study, an automated detection system of malaria parasites from thick blood smear of microscopic image with low quality that obtained from endemic regions in Indonesia has been built.
The proposed method consists of three main stages. The first stage is preprocessing the low quality of thick blood smear image because the presence of parasites is not easily legible. At this stage, image enhancement uses edge correction based on dual contrast enhancement to increase contrast and sharpen the line of objects. Secondly, segmentation and classification of blood components to obtain parasite candidates from thick blood smear images using intensity slicing, morphological operations, and feature-based rules of the parasitic nucleus. Thirdly, identification of parasite candidates may decrease the false positive in determining the status of malaria parasites. This stage consists of segmentation process using Watershed, Index of Fuzziness, and Adaptive Thresholding and utilization of parasitic features to determine its status.
The performance was evaluated using the thick blood smear of microscopic image from the Eijkman Institute for Molecular Biology. The results showed that 1) the proposed enhancement method achieved the highest entropy average that is 7.03 with an increase of 23% compared to the image before enhancement, so it could produce images with more information; 2) the proposed method for segmentation of parasite candidate achieved mean of precision, recall, and F-measure were all 86%. The used of parasite nucleus features in the classification of blood components improved the precision of the system; 3) the method of identification of parasite candidate by utilizing the features of parasite was able to decrease false positive greater than 70% compared to the system result in the stage of segmentation and blood component classification. Based on these results it can be concluded that the proposed method is appropriate to be used for malaria parasites detection.
