Abstract

Penelitian di bidang biomekanik terkait implan ortopedik sedang
berkembang pesat di dunia, khususnya untuk fiksasi patah tulang. Fiksasi yaitu
pembedahan untuk menempatkan dan melekatkan pelat (plate) logam yang
diperkuat dengan sekrup (screw) pada sekitar tulang yang patah. Tulang yang
rentan patah yaitu tulang paha (femur) yang mayoritas disebabkan oleh
kecelakaan kendaraan bermotor. Data terbaru yang dikeluarkan oleh World
Health Organization (WHO) menunjukkan Indonesia menempati urutan kelima
kasus kecelakan kendaraan bermotor di dunia. Data dari Kepolisian Republik
Indonesia tercatat sepanjang tahun 2013 terjadi 93.578 kasus kecelakaan
kendaran bermotor. Hampir 80% dari korban kecelakaan tersebut menderita
patah tulang. Oleh karena itu perlu dilakukan penelitian terkait implan ortopedik
dan fiksasi patah tulang yang kompatibel dengan masyarakat Indonesia. Sebelum
memproduksi plate dan screw yang cocok dengan kondisi tulang masyarakat
Indonesia, maka sebelumnya diperlukan pemodelan dan simulasi terlebih dahulu.
Agar kompatibel dengan masyarakat Indonesia, maka pemodelan dilakukan
dengan menggunakan tulang femur dari ras Mongoloid. Jenis material yang
dapat digunakan untuk membuat screw adalah logam stainless steel.
Kelebihannya adalah biokompatibel cukup baik, murah, dan ulet. Penelitian ini
difokuskan untuk pemodelan dan simulasi screw menggunakan logam SS316L dan
SS304 untuk fiksasi tulang femur menggunakan perangkat lunak ANSYS 15.0.
Variabel yang akan dianalisis adalah diameter screw, pemilihan jenis material,
serta jumlah dan posisi screw. Variabel-variabel tersebut sangat mempengaruhi
fiksasi internal terhadap kestabilan dan kekuatan biomekanik di dalam tulang.
Respon yang diperoleh berupa tegangan, regangan, dan total deformasi dari
masing-masing tulang dan screw. Berdasarkan penelitian didapatkan hasil desain
optimal screw adalah untuk diameter 4,5 mm dengan kombinasi jenis plate slotted
dan material penyusun SS316L karena memiliki nilai tegangan von Mises,
regangan, serta deformasi terkecil dan berada di bawah nilai tegangan luluh
material.

Research in the field of biomechanics related orthopedic implants is
rapidly growing in the world, especially for fixation of fractures. Fixation is a
surgery to locate and attach the plate reinforced with metal screws at about a
broken bone. Most of broken bones sited in thigh bone (femur) that the majority
are caused by motor vehicle accidents. The latest data released by the World
Health Organization (WHO) shows that Indonesia is the fifth case of motor
vehicle accidents in the world. Data from the Indonesian National Police
recorded during the year 2013 happened 93.578 cases of motor vehicle accidents.
Almost 80% of the accident victims suffered broken bones. Therefore, it is
necessary to research related to orthopedic implants and fixation of fractures that
are compatible with the Indonesian people. Before producing the plate and screws
that match the bone condition of Indonesian society, it previously required
modeling and simulation beforehand. To be compatible with the Indonesian
people, the modeling is done by using the femur of the Mongoloid race. Types of
materials that can be used to make the metal screw is stainless steel. The surplus
is good biocompatibility, cheap, and tenacious. This research focused on
modeling and simulation using a metal screw SS316L and SS304 for the fixation
of the femur using ANSYS 15.0. Variable that will be analyzed is the screw
diameter, material selection, number and screw position. They are affect in
internal fixation by stability and strength in the bone biomechanics. Responses
obtained are stress, strain, and total deformation. It is obtained the optimum
screw diameter is 4,5 mm combined with slotted plate and the best material is
SS316L due the value of von Mises stress below the yield strength of material,
minimum value of strain and deformation.
