Abstract
Failure Modes and Effects Analysis (FMEA) merupakan sistem evaluasi potensi kegagalan yang umum digunakan pada perusahaan untuk mengambil keputusan terakait dengan prioritas penggantian peralatan dan menetapkan kapan penggantian peralatan tersebut. Namun, metode FMEA tidak mencantumkan bobot pada setiap butir penilaiannya yang mengikuti konteks obyek evaluasi serta besaran ketidakpastian dari penilaian. Modifikasi yang digunakan adalah aplikasi Dempster Shafer Theory yang dapat menyatukan pendapat penyematan nilai beban kontekstual dari anggota tim penyusun FMEA berdasarkan faktor kegagalan setiap bagian sistem dan ketidakpastiannya. Hasil modifikasi ini bernama Generalized Evidental FMEA (GEFMEA), yang hasilnya telah merombak urutan prioritas dari FMEA konvensional. Komponen dari sistem turbin turbofan obyek penelitian yang berpotensi mengalami kegagalan fatal akan didahulukan dengan memperhatikan bobot setiap faktor kegagalan berdasarkan konteks. Peringkat pertama pada GEFMEA yang di duduki oleh failure mode 12 terdapat rekristalisasi dari bilah rotor dalam ukuran mikrosopis yang akan mengakibatkan kegagalan mulur dari material yang diseababkan oleh stress dan vibrasi. Dengan nilai total GERPN sebesar 68,94227 kenaikan peringkat pertama dari posisi 3 disebabkan oleh penambahan nilai kontekstual kerumitan identifikasi proses rekristalisasi dan ketidakpastian. Pada posisi 2 terdapat failure mode 9 dengan nilai 68,94227, dan failure mode 14 dengan nilai 44,37769 pada posisi 3. Pada dasar tabel GEFMEA, posisi 15, 16, dan 17 tidak mengalami perubahan karena tidak berakibat fatal dan mudah teridentifikasi oleh inspeksi berkala.

Failure Modes and Effects Analysis (FMEA) is a potential failure evaluation system commonly used in companies to make decisions regarding equipment replacement priorities and determine equipment replacement timeline. However, the FMEA method does not include a contextual mass on each of its assessment items which follows the context of the evaluation object as well as the uncertainty magnitude of the assessed system. The modification used is the Dempster Shafer Theory application which can unify the appraisal of embedded contextual load values from each member of the FMEA team based on the failure factors of each part of the system and its uncertainties. The result of this modification is called Generalized Evidental FMEA (GEFMEA), whose results have overhauled the priority order of conventional FMEA. Components of the turbofan turbine system as the research object that have the potential to experience fatal failure will be prioritized by considering the weight of each failure factor based on the context. The first rank in GEFMEA which is occupied by failure mode 12 where the recrystallization of the rotor blades in microscopic sizes which will result in creep failure of the material due to stress and vibration. With a total GERPN value of 68.94227, the increase in first rank from position 3 was due to the addition of contextual values for the complexity of identification of the recrystallization process and uncertainty. In position 2 there is failure mode 9 with a value of 68.94227, and failure mode 14 with a value of 44.37769 at position 3. On the basis of the GEFMEA table, positions 15, 16, and 17 do not change because they are considered not fatal and are easily identified by periodical inspection.
