Abstract

Akses jaminan kesehatan bagi nelayan masih menghadapi tantangan, terutama pada segmen Peserta Bukan Penerima Upah (PBPU) yang dituntut membayar iuran mandiri di tengah pendapatan yang fluktuatif dan kondisi pekerjaan yang berisiko tinggi. Rendahnya partisipasi PBPU dalam kepesertaan mandiri JKN membuat perlu dianalisis sejauh mana nelayan memiliki kemampuan dan kemauan untuk membayar iuran secara sukarela. Penelitian ini bertujuan untuk menganalisis faktor-faktor yang memengaruhi Willingness to Pay (WTP) nelayan Sukolilo Baru terhadap iuran Jaminan Kesehatan Nasional (JKN) pada segmen Peserta Bukan Penerima Upah (PBPU). Sebanyak 52 responden dipilih melalui teknik purposive sampling dan dianalisis menggunakan regresi logistik biner. Variabel independen disusun berdasarkan indikator kesejahteraan rumah tangga menurut BPS, meliputi pendapatan total rumah tangga, pengeluaran non-kesehatan, pengeluaran berobat total, kondisi kesehatan, frekuensi pembelian obat mandiri, frekuensi kunjungan ke fasilitas kesehatan umum, lokasi fasilitas kesehatan rujukan, pendidikan, jumlah anggota rumah tangga yang bekerja, kondisi tempat tinggal, jumlah anggota rumah tangga, dan kepemilikan kartu JKN Responden. Hasil penelitian menunjukkan bahwa rata-rata Ability to Pay (ATP) sebesar Rp310.032, sedangkan rata-rata WTP hanya Rp15.865. Meskipun sebagian nelayan secara finansial mampu membayar iuran JKN kelas 3, sebagian tidak bersedia membayar secara mandiri, sehingga terjadi kesenjangan antara ATP dan WTP. Analisis statistik menunjukkan bahwa pengeluaran non-kesehatan, kondisi kesehatan, lokasi fasilitas kesehatan rujukan, pendidikan, kepemilikan kartu JKN berpengaruh signifikan terhadap WTP (p < 0,10). Nilai Nagelkerke R² sebesar menunjukkan bahwa model mampu menjelaskan variasi WTP.

Access to health insurance for fishermen still faces challenges, especially for the Non-Wage Recipient Participants (PBPU) segment, who are required to pay contributions independently amid fluctuating incomes and high-risk working conditions. The low participation of PBPU in voluntary JKN membership necessitates an analysis of the extent to which fishermen have the ability and willingness to pay contributions voluntarily. This study aims to analyze the factors influencing the Willingness to Pay (WTP) of Sukolilo Baru fishermen regarding National Health Insurance (JKN) contributions in the Non-Wage Recipient (PBPU) segment. A total of 52 respondents were selected through purposive sampling technique and analyzed using binary logistic regression. Independent variables were compiled based on household welfare indicators according to BPS, including total household income, non-health expenditures, total medical expenditures, health conditions, frequency of independent drug purchases, frequency of visits to public health facilities, referral health facility location, education, number of employed household members, living conditions, number of household members, and JKN card ownership. The results showed that the average Ability to Pay (ATP) was Rp310,032, while the average WTP was only Rp15,865. Although some fishermen were financially able to pay JKN class 3 contributions, some were unwilling to pay independently, resulting in a gap between ATP and WTP. Statistical analysis shows that non-health expenditures, health conditions, location of referral health facilities, education, ownership of a JKN card have an effect significantly influence WTP (p < 0.10). The Nagelkerke R² value indicates that the model is able to explain of the variation in WTP
