Abstract
Pembangunan infrastruktur Sistem Penyediaan Air Minum (SPAM)
merupakan salah satu proyek strategis nasional yang memiliki tingkat kompleksitas teknis dan tata kelola yang tinggi. Dalam pelaksanaannya, proyek SPAM kerap menghadapi permasalahan Contract Change Order (CCO) yang berdampak pada perubahan biaya dan lingkup pekerjaan. Penelitian ini bertujuan untuk mengidentifikasi faktor-faktor teknis yang berkontribusi terhadap terjadinya CCO, menganalisis struktur hubungan logis antar faktor penyebab menggunakan metode Fault Tree Analysis (FTA), serta merumuskan rekomendasi pencegahan berbasis prioritas sebagai lessons learned untuk proyek SPAM atau proyek infrastruktur sejenis di masa mendatang. Penelitian ini menggunakan pendekatan studi kasus pada proyek SPAM Regional Wosusokas dengan memanfaatkan data primer berupa expert judgment dari para pengambil keputusan CCO dan data sekunder berupa dokumen kontrak, berkas CCO, notulen teknis, serta laporan progres proyek. Analisis dilakukan melalui pemodelan FTA untuk mengidentifikasi kejadian dasar (basic events), struktur kegagalan, minimal cut set, serta probabilitas kejadian yang memicu terjadinya CCO teknis. Hasil penelitian menunjukkan bahwa faktor Lapangan dan Kondisi Eksisting, Material dan Logistik, serta Owner/Pemilik merupakan faktor dominan penyebab CCO, baik dari sisi frekuensi kejadian maupun dampak biaya. Pemodelan FTA memperlihatkan bahwa struktur kegagalan didominasi oleh gerbang logika OR dengan first order cut sets, yang mengindikasikan bahwa banyak kejadian dasar secara mandiri sudah cukup untuk memicu terjadinya CCO teknis. Berdasarkan temuan tersebut, dirumuskan rekomendasi pencegahan yang menitikberatkan pada penguatan investigasi lapangan, pengetatan sistem QA/QC perencanaan, pemutakhiran dokumen perencanaan, serta perbaikan tata kelola pemilik melalui integrasi perencanaan dan operasi serta penerapan prinsip Life Cycle Costing.

The development of Water Supply System (WSS) infrastructure constitutes one of Indonesia’s national strategic projects, characterized by high levels of technical complexity and governance challenges. In practice, WSS projects frequently encounter Contract Change Orders (CCOs), which result in changes to project costs and work scope. This study aims to identify the technical factors contributing to the occurrence of CCOs, analyze the logical structure of relationships among these factors using Fault Tree Analysis (FTA), and formulate priority-based preventive recommendations as lessons learned for future WSS projects or similar infrastructure developments. This research adopts a case study approach focusing on the Regional Wosusokas WSS project, utilizing primary data in the form of expert judgment from key CCO decision-makers and secondary data derived from contract documents, CCO records, technical meeting minutes, and project progress reports. The analysis is conducted through FTA modeling to identify basic events, failure structures, minimal cut sets, and the probabilities of events triggering technical CCOs. The results indicate that field and existing conditions, material and logistics, and owner-related factors are the dominant contributors to CCOs in terms of both occurrence frequency and cost impact. The FTA model reveals that the failure structure is predominantly governed by OR logic gates with first-order cut sets, indicating that many basic events individually are sufficient to trigger technical CCOs. Based on these findings, preventive recommendations are formulated emphasizing enhanced field investigations, stricter design QA/QC systems, continuous updating of planning documents, and improved owner governance through the integration of planning and operations as well as the application of Life Cycle Costing principles.
