Abstract

Jurnal ini dilatar belakangi dengan permasalahan yang pada umumnya terjadi pada sistem pipa bawah laut (offshore pipeline) yang menyebabkan kegagalan operasi akibat kerusakan pada pipa pada saat beroperasi, kerusakan yang terjadi pada pipa bawah laut pada umumnya terdata atau terdokumentasi pada saat dilakukan inspeksi rutin. Hasil inspeksi terdokumentasi beberapa faktor yang dapat menjadi kegagalan; ada jangkar yang menyangkut, coating yang terkupas, tanda-tanda korosi pada pipa. Untuk melakukan peneltian maka untuk mengidentifikasi kegagalan yang dapat terjadi pada pipa bawah laut untuk mengalami kerusakan maka digunakan metode fault tree analysis (FTA) secara top down untuk mengetahui top event utama hingga basic event, untuk mengetahui indeks probabilitas dan keparahannya maka metode cut set (untuk mendapatkan probabilitas top event) dan metode what if (untuk mengetahui dampaknya), setelah mendapat angka maka hasil per kegagalan dapat dimasukkan kedalam matriks resiko untuk mengetahui tingkat risiko dan untuk menentukan penanganannya digunakan metode HAZOP (Hazard and Operability) Analysis. Dari hasil penelitian teridentifikasi 60 event yang menyebabkan kerusakan pipa tersebut dimana coating terkelupas dan korosi menjadi kegagalan yang memiliki probabilitas dan keparahan terbesar serta penanganan untuk kegagalan yang telah diidentifikasi.==========================================
This thesis will tackle the problem(s) regarding a situation that normally occur to a subsea pipeline system that could cause a breakage or certain amount of damage to the pipe (itself) during operation, the damage(s) that (could) happen to a (subsea) pipeline are documented during a routine inspection. The result(s) of the inspection (according to a report by said company) found few failures that could be identified as the factors that cause the damage to the pipeline; an anchor that got stuck to the pipe, a peeled coating, and signs of corrosion on the pipe. To further analyze this problem, first the failure must be identified to a certain extent mainly that could cause a certain amount of damage to the pipeline, fault tree analysis is used a method to identify these failures, a top down approach is used to identify the top event all the way to the basic event, to obtain the probability and severity index , cut set and what if analysis methods are used to obtain the probability and effect of each failure (respectively). The results are then analyze by using risk matrix to obtain the risk level of each failure, as for how to decide the (preventive) action for each failure, HAZOP analysis is used as method for that matter. From the results of the analysis, 60 failures (event(s)) are identified to have the potential to cause damage to the pipeline with a peeled coating and corrosion as the failures that have the highest probability and severity and also a preventive action for each failure that have been identified.
